DEACTIVATE ACCOUNT
The FourC Health app serves as a portal providing access to your legal medical records, which are maintained by healthcare providers and staff at FourC Health, Inc. Due to legal and regulatory requirements governing the storage of medical records, we are unable to delete your account or its contents. However, we can deactivate your account as outlined below.
To request deactivation:
Open the FourC Health app
Select the Messages icon from the bottom navigation bar
Navigate to the Customer Support tab at the top
Send a message requesting the deactivation of your account.
Once the request is processed, you will need to log out of the app for the deactivation to take effect. If the request is made outside the app, such as via email, identity verification may be required. Please note that deactivating your account will result in the loss of access to the FourC Health app.
