What is direct primary care?

Direct primary care is a healthcare model where you pay your doctor directly for a defined set of primary care services, without involving your health insurance. Instead of paying for each visit or service, you typically pay a predictable monthly or annual membership fee. Any services obtained outside the practice, such as tests, medications, procedures, or visits to other facilities are your responsibility but can often be covered by your insurance, if applicable.


How are you different from concierge care service?

We are very similar to concierge care, with the key difference being affordability. Our goal is to address your medical needs as efficiently as possible, without charging per visit or billing your insurance like some concierge practices do. We also do not offer in-clinic lab services or medications at this time, which helps keep our services more affordable.


What services are included?


What services are excluded?

The following services are excluded: services provided by third parties or hospital services, emergency room visits, or urgent care facilities visits; visits with other providers or specialists, whether referred by us or otherwise; radiology services; lab tests done by external companies; durable medical equipment; prescription medications; other ancillary services like physical therapy or any other services that are not specifically listed as included services.


Why do you need a 1 year commitment?

We believe that doctor-patient relationships in primary care should be long term with no barriers or financial considerations during every single interaction. It would be difficult to judge the value of your doctor’s services or the membership in just a few months. When the doctor knows that you are committed to stay with them long term, they know to prioritize your long term health goals rather than providing short term solutions. This will also ensure that your doctor is available to you whenever you need them. Without this commitment period, appointments and the doctor’s time would be booked on a first-come, first-serve basis, which would lead to long wait times similar to traditional practices. However, if you are unsatisfied with our services for any reason and want to cancel your membership before the commitment period, please contact customer support through the FourC Health app and we will make it right for you.

Medical Services Questions

Why do you require the first visit to be in-person rather than through video?

We believe that an in-person interaction during the first visit is essential for building rapport, trust, and a strong doctor-patient relationship. It allows for a thorough baseline physical examination and sets the foundation for long-term care. After the initial visit, you can choose the care method that’s most convenient for you or medically necessary, whether that’s in-person, video, phone consultation or messaging.


Do you treat obesity or prescribe weight loss medications?

Yes, we manage obesity and provide weight loss treatment as part of our comprehensive care. While we are not an exclusive weight loss clinic, we manage obesity like other chronic medical conditions that a primary care doctor would manage. Some insurance companies may require the prescription of certain controlled substances, such as phentermine, before approving newer GLP-1 medications like Wegovy and Zepbound. However, we are not currently set up to e-prescribe controlled substances.


Do you submit prior authorizations?

Yes, if medically necessary


Do you see children?

We do not see patients under the age of 18 at this time.


Why don’t you prescribe controlled substances?

Controlled substances require electronic prescriptions, and our current e-prescription system is not yet set up to handle them. Since we are a newly launched practice, we are working on adding this feature in the future to better serve our patients. 


Why don’t you administer vaccines in your clinic?

Vaccinations are typically covered by most insurances, and for those without insurance, they are available at competitive prices through pharmacy chains or local health departments. As we do not bill insurance, it is more cost-effective for patients to receive vaccines at these locations.


Do you practice integrative or alternative medicine?

While we emphasize evidence-based holistic, preventive, and wellness measures as part of your overall health management, we are not specialized in integrative medicine. We do not prescribe or sell alternative healthcare products

Insurance Questions

Do you accept health insurance?

No, we do not accept health insurance and we do not bill your insurance company for any services provided


Why don’t you accept Medicare or Medicaid or patients with other government insurance like Tricare?

Doctors may maintain enrollment with Medicare or Medicaid or Tricare for hospital privileges or to provide care in other facilities. However, billing regulations prohibit enrolled doctors from charging Medicare or Medicaid or Tricare patients directly for covered services like appointments, as these must go through insurance. Since we do not bill insurance for any of our services and cannot bill patients directly due to these regulations, we are unable to accept Medicare or Medicaid patients or those with other government insurance like Tricare at this time.


Does this service replace or provide insurance?

No, this service or membership is not an insurance product and we are not an insurance company. We recommend that you obtain health insurance separately.


Do I need health insurance to subscribe to the service?

No, you do not need health insurance to subscribe to our services. While we recommend having insurance for comprehensive coverage, we understand that some people may not have insurance for various reasons, and we are happy to serve you regardless.

Billing/Payment Questions

What forms of payment do you accept?

We currently accept credit card payments only. At this time, we do not support other payment methods such as cash, checks, ACH transfers, Apple Pay, Google Pay, PayPal, Venmo, or Zelle.


Does the price include tax?

No. In New Mexico, gross receipts tax (similar to sales tax) applies to payments for medical services. While it is possible for us to absorb this cost, doing so would not be financially sustainable given our already low membership fees. Instead, we pass the tax on to you to ensure we have the resources to continue providing high-quality care at an affordable price. We strive to remain transparent about our pricing and still offer exceptional value compared to traditional healthcare models.


Can I use my HSA account to pay for the membership? Will membership fees apply to my deductible?

You can use your HSA account to pay for the membership, as it covers professional medical services. However, payments will not apply to your deductible, as we are out-of-network and do not bill or submit claims to your insurance.


Do you accept payment in the clinic?

No, we do not accept payments in the clinic. All payments should be made when you subscribe to our service through the FourC Health app.


Do I have to pay for lab tests, radiology, prescriptions, visits or admission to another clinic or facility or hospital or specialist?

Yes, you are responsible for any charges for services provided outside our clinic or by anyone other than our doctors or staff. You can use your insurance for these services, if applicable.
